Preheat the oven to 375 degrees and lightly grease a loaf pan.
In a large bowl, combine the ground beef, half the breadcrumbs, half the Parmesan cheese, half the minced garlic, the egg, milk, onion, ketchup, salt, and pepper. Mix until evenly blended but do not overwork the meat.
Press the mixture firmly into the prepared loaf pan to form an even shape.
In a small bowl, stir together the remaining breadcrumbs, remaining Parmesan cheese, and remaining minced garlic. Sprinkle this mixture evenly over the top of the meatloaf to create the crust.
Bake for 50 to 55 minutes until the internal temperature reaches 160 degrees and the crust is golden.
Remove from the oven and let the meatloaf rest in the pan for 10 minutes before slicing and serving.
