Heat the oven to 375 degrees and line a baking sheet with parchment paper.
In a large bowl, combine the ground beef, diced apple, breadcrumbs, egg, onion, smoked paprika, salt, pepper, and 2 tablespoons of the BBQ sauce. Mix with your hands until evenly blended.
Shape the mixture into a loaf about 8 inches long and place it on the prepared baking sheet.
In a small bowl, stir together the maple syrup, remaining 2 tablespoons BBQ sauce, and apple cider vinegar to make the glaze.
Brush half the glaze over the top and sides of the meatloaf.
Bake for 35 minutes, then brush with the remaining glaze.
Continue baking for 15 to 20 minutes more until the internal temperature reaches 160 degrees. Let the meatloaf rest for 5 minutes before slicing.
