Preheat the oven to 350 degrees and line a baking sheet with parchment paper.
In a large bowl, combine the ground beef, chopped mushrooms, onion, garlic, breadcrumbs, egg, milk, thyme, salt, and pepper. Mix gently with your hands until just combined.
Shape the mixture into a loaf about 8 inches long and place it on the prepared baking sheet.
Spread the ketchup evenly over the top of the loaf.
Bake for 55 to 60 minutes until the internal temperature reaches 160 degrees. Let the meatloaf rest for 10 minutes before slicing.
